The overhead cable tricep extension is the cable exercise for the back of your upper arm. You face away from the machine, hold a rope behind your head, and straighten your arms overhead.
Only the forearms move. The upper arms stay up beside your ears.
Watch three people do it in the same gym and you will see three setups: standing tall at a low pulley, leaning forward at a mid pulley, bent nearly double under a high pulley. Below is how to set up, how to do the rep, which of the three to use, and how much weight to start with.

Why Do the Overhead Cable Tricep Extension?
The long head of the triceps attaches to the shoulder blade, so it is only stretched when the arm is raised. A pushdown keeps the arm at your side and never stretches it. This exercise loads it at full length on every rep, which is the position a muscle grows from fastest. If your arms have stopped responding to pushdowns, this is usually what is missing. How to Do the Overhead Cable Tricep Extension
Step 1: Set the pulley
Set the pulley at about shoulder height, or a little below. Not at the very bottom. When the pulley is at the bottom of the column, the cable comes up at such a steep angle that you have to wrestle the rope into position before the first rep, and at the stretch the cable pulls your hands down instead of back. At shoulder height the cable runs roughly in line with your forearms at the bottom of the rep, which is where you want the load.
If your machine has only a fixed high and low pulley, use the high one and bend forward more at the hips. If it has only a low pulley, use it and stand a step further from the machine.
Step 2: Attach the rope
Use the rope. A straight bar locks your wrists in one position at a point in the movement where they want to rotate; the rope lets them turn. An EZ-bar attachment is the second choice if you want a fixed handle. For one arm, use a single D-handle.
Step 3: Set up
Grab the rope with both hands, palms facing each other, thumbs on top of the knots. Turn so your back is to the machine. Bring the rope up and over so your hands are behind your head.
Step one foot forward into a staggered stance, the front knee soft, the back foot planted. Walk forward until there is tension on the cable with your arms bent. The cable should now run from the pulley up over your shoulder to your hands, touching nothing.
Brace your abs as if someone were about to poke you in the stomach, and pull your ribs down. This is what stops your lower back from arching when the weight pulls your arms back.
Step 4: Find your elbow position
This is the step that decides whether the exercise works. Your elbows should be up beside your ears, pointing forward, and close together. Not out to the sides, and not down in front of your face.
Whatever angle your torso is at, the upper arms stay in line with it. If you are standing tall, the upper arms point straight up. If you are leaning forward at the hips, the upper arms point forward-and-up with the torso. What you must not do is let the elbows drop in front of your head while the torso stays upright; that takes the stretch off the long head and turns the exercise into a pushdown done at a strange angle.
Step 5: The rep
Breathe out and straighten your arms, driving the rope up and slightly forward until the elbows are almost locked. As your hands pass the top, let them separate a few inches so the rope ends finish apart. Squeeze the back of your arms for one second.
Breathe in and let the cable pull your hands back behind your head slowly, two to three seconds, until your forearms are past parallel with the floor and you feel a clear stretch down the back of your upper arm. Pause there for a moment. Do not let the elbows flare out as the hands come down.
Then drive back up. The elbows do not move forward, backward, or outward at any point; if you filmed the rep from the side, the upper arms would look pinned in place while the forearms swing.
Step 6: Finish the set
Do not drop the stack. Bring the rope back to the stretched position under control, then walk back toward the machine to take the tension off before you let go.
Pulley Height and Body Angle
All three setups work if the elbows stay beside the ears. Here is what changes.

Our default is the second one. The test for any of them: at the bottom of the rep, are your elbows level with or above your ears, relative to your torso? If yes, the long head is stretched. If they have drifted to face level, it is not.
Overhead Cable Tricep Extension Mistakes
Elbows dropping in front of the head. The stretch comes off the long head and it becomes a backward pushdown. Lower the weight, or lean forward more so the torso meets the arms instead of the arms coming down to meet the torso.
Elbows flaring out. Shoulders and chest take the bottom of the rep, and the elbow joint takes the strain. Imagine the movement inside a tube the width of your head; keep the rope ends close together at the bottom.
Arching the lower back. The stretch you feel is in the back, not the triceps. Brace, ribs down, slight forward lean. If it still arches with the pulley low, move it up to shoulder height.
Cutting the stretch short. Hands come down only to the top of the head and go straight back up, skipping the reason to do this exercise. Lower until the forearms pass parallel with the floor and pause there; if you cannot get there, the weight is too heavy.
Swinging. The torso rocks and the body moves the stack. Set the stance, brace, freeze the torso, and let only the forearms move. If the weight rocks you off balance, take a plate off.
Variations
Same rule for all three: upper arm beside the head, only the forearm moves.
Single-arm, with a D-handle. Each arm finds its own path and you see which side is weaker. About half the two-arm weight; start with the weaker arm and match its reps on the other.
Seated, on a bench with its back to a low pulley. The back pad takes away the balance and stops the lower back arching. Use it if the standing version turns into a back exercise.
Cross-cable, on a functional trainer. Hold the left cable in the right hand and the right in the left, so the cables cross behind your back and each arm gets its own line of pull. Roughly half the rope weight per side.
Sets, Reps, and Weight
Start at about two-thirds of the weight you use for a rope pushdown, then adjust until you can do 12 clean reps with the elbows staying put. Check the pulley ratio on the machine placard before comparing numbers between machines; we covered why in the pushdown guide.
3 sets of 10 to 15, 60 to 90 seconds rest. One second up, one second squeeze, two to three seconds down with a pause at the stretch. Once or twice a week, after pressing and before pushdowns if you do both in one workout. When you hit 15 on all sets with the elbows pinned, go up one plate and start again at 10 to 12.
Equipamiento
The one thing this exercise needs from the machine is a pulley that can be set at shoulder height.
